This release is entered into between the undersigned and BKWellness Enterprises LLC, its officers, members, subsidiaries, affiliates , agents, assigns, employees, contractors, subcontractors, administrators, and executors. The purpose of BK Wellness Enterprises LLC is to provide fitness and wellness instruction, coaching and training for various levels of individuals. The purpose of the Choose Well to Live Well “Empower Yourself” online course and membership site is to provide comprehensive training and coaching to improve health and wellbeing.
The undersigned hereby agrees to and acknowledges that:
Laura Boniello-Krout is not a physician and is not trained in any way to provide medical diagnosis, medical treatment or any other type of medical advice;
The intent of Laura Boniello-Krout is only to offer coaching, training and instruction of a general nature to help you in your quest for emotional, physical and spiritual well- being;
Coaching, training and instruction is another tool for teaching individuals about themselves, but that BK Wellness Enterprises does not guarantee immediate results, or good/bad results;
Should the undersigned experience any discomfort, pain, or feel out of the ordinary during training, the undersigned should contact a physician at once and the undersigned should always get medical approval before starting any exercise program;
Pilates, Yoga, total body workouts, relaxation session, roller release, weight training, boot camp, can be uncomfortable and tests one’s mental and physical limits and carry with it potential for injury therefore the undersigned assumes the risks of participating in these types of activities this membership site provides.
The undersigned expressly waives, release, discharges from any liability of personal injury or action of any kind and agrees not to sue or otherwise commence any action of any kind against BK Wellness Enterprises LLC, in consideration for the undersigned participating in this Fitness and Wellness coaching membership, or any other BK Wellness Enterprises, LLC, program or activity.
The undersigned agrees that this is the full agreement between BK Wellness Enterprises LLC , and that the undersigned has entered into this agreement voluntarily.
